In case anyone is interested, you can download a paper that will appear in
the
Journal of the British Interplanetary Society (JBIS) either this month or
the
next on software safety for spacecraft.

The new hazard analysis method described is being used on the U.S.
Missile Defense System, Orion (the Space Shuttle replacement) and is
being evaluated on the HTV by JAXA. It is also being used in non-space
and non-military applications. I have two grant proposals out right now
to try it on automotive applications. We've applied it to auto systems as
exercises, but not evaluated it formally in this application nor carefully
compared it to the results of using other approaches outside aerospace.
